Healthy Starts

Healthy Start is voluntary home visiting program designed to improve the lives of children and their families.

Home visitors will make regularly scheduled visits providing education and support to help parents be the best they can be.

The program is offered to any pregnant woman (regardless of age), newborn and infant up to age 5 in Greeley, Hall, Hamilton, Howard, Merrick and Sherman counties who meet the screening criteria.

The program is staffed by Registered Nurses and LPNs staff the family education component. Bilingual professional staff is available.

Healthy Starts will help:

  • By working with the expectant mother to help her understand the prenatal development and growth of her child. Home visits will be made by a qualified professional to assist parents in the preparations for the hospital, delivery and the newborn's homecoming.
  • By making regular home visits after the birth to promote and enhance positive parenting skills.
  • By providing education to parents on their child's growth and development.
  • By making sure the home is a safe and accident-free environment for young children.

Healthy Starts will assist parents in:

  • Creating a healthy and nurturing home atmosphere for their child.
  • Coping with and managing stressful situations that may arise in the home.
  • Contacting educational or career-oriented placement services to help families reach their economic goals.
  • Contacting other area agencies to assist the family in overcoming obstacles they may encounter and strengthen the family unit as a whole.

Also available is a seven-week Parenting Course focused on children from birth to age 5.

What is Home Health Care?

Home Health Care Services provides a continuity of nursing and medical care, in the Christian spirit of service, to patients after they leave the hospital. In some cases, patients are able to be discharged from the hospital at an earlier date knowing that their health needs will be met in their home. Home Health helps to prevent hospitalization by giving care to patients who become ill in their homes.

Saint Francis Medical Center's Home Health Care program complements existing health services in the area. The staff cooperates with other community organizations and agencies in obtaining services, financial assistance, personnel and equipment for patient care.

Who Can Refer a Patient?

Referrals are accepted from physicians, hospitals, families, patients, friends and interested agencies. Hospitalized patients should be referred prior to dismissal whenever possible for a smooth transition. However, referrals may be made at any time.

What Does Home Health Provide?

Saint Francis Medical Center Home Health Care provides the following services in the home under the direction of a physician:

Intermittent Care:

  • Nursing Care
  • A Registered Nurse is on call 24 hours a day.
  • High Tech Procedures
  • Home Health Aides
  • Physical Therapy
  • Speech Therapy
  • Occupational Therapy
  • Medical Social Worker

Home Care Objectives:

  • To lessen the effect of an illness on the patient and his/her family by maintaining personal dignity in the patient's own home.
  • To treat the patient with respect, preventing or minimizing physical, mental and emotional illness.
  • To provide continuity of nursing and medical care.
  • To meet the need for healthcare leading up to, in place of, or following hospitalization.
  • To help reduce or minimize the length of stay in the hospital.
  • To speed the rehabilitation of the patient.
  • To plan and implement a teaching plan for the patient to prevent further illness.
  • To involve the family/ caretaker in personal care, health planning, teaching and follow-up care.

Who is Eligible?

Home Health Care is available to all persons regardless of age, handicap, race, sex, religion or national origin.

What Does it Cost?

Saint Francis Medical Center Home Health Care is a non-profit organization.

Coverage of Services:

Your physician is in charge of the plan of care and frequency of visits by the Home Health Care staff. Patients reside in our service area of Hall, Hamilton, Howard, Merrick, Sherman, Greeley, Nance and Boone Counties, as well as parts of Buffalo, Madison, Platte, Valley and Wheeler counties.

Home Health Office Hours are 8 a.m.-4:30 p.m., Monday through Friday. After 4:30 p.m., call (308) 384-4600 and ask that the Home Health nurse on call return your call.
